The applicant should download and complete this form - English or Chinese . If the applicant is under 18, a parent or guardian should complete the form.
Please print clearly in BLOCK LETTERS and tick the appropriate box. Your name on the course certificate will be typed as it appears on the enrolment form.
Each form is considered valid only with complete information and the applicant's signature on it.
The completed Enrolment Form and a HK$800 deposit should be sent to Outward Bound Hong Kong, Tai Mong Tsai, Sai Kung, Hong Kong.
